Documents issued in Poland
IMPORTANT
- Failure to submit paper documents b the deadlines will result in non-enrollment on the student list;
- A candidate not qualified for admission is not entitled to submit documents;
- Documents may be submitted in person, by a third party, or sent by post or courier (the date of delivery, not the date of posting, is what counts). We do not require authorisation for submission of documents by a third party;
- The submission of documents by persons with “reserve” status does not mean being admitted. A “reserve” candidate takes the place of a “qualified” person (according to the ranking) in the event that this person has not provided a complete set of documents by the set deadline, has resigned from the recruitment process or has been disqualified.
Candidates for first cycle studies
Scans – to be submitted via IRK system
- Photo
- Project- in case of candidates for Individual Interdisciplinary Studies
- In the case of winners or finalists of Polish olympiads – a document issued by the main organizing committee of a given olympiad or competition, bearing a serial number, confirming the place taken by a given candidate for studies in the central qualifying round;
- In the case of athletes with outstanding achievements in Olympic disciplines and those included in the programme of the Polish Academic Championships – a document issued by a Polish sports association (in the case of the master class and the first class) or a district/voivodeship sports association (in the case of the second sports class), specifying the type of sports class, its validity period and the sports achievements on the basis of which it was awarded;
- NAWA decision granting scholarship – NAWA scholars only;
Paper documents
Candidates with ‘QUALIFIED’ or ‘RESERVE’ status who have received their education in Poland must deliver the original paper documents listed below within 4 days after the announcement of the list of candidates to the Admission Committee
- Self-signed personal questionnaire with photo generated and printed from the IRK system
- Polish matura certificate or International Baccalaureate or European Baccalaureate. Candidates who took the International Baccalaureate in 2026 and are awaiting the issuance of a diploma must submit a certificate of their International Baccalaureate results (issued by the IB Office in Geneva or the school they graduated from) qualifying them for the issuance of a diploma. A copy of the diploma (original available for review) must be submitted to the relevant dean’s office no later than October 1, 2026;
- Passport – in case of foreigners
- Legal representative statement of consent- in case of candidate under 18
- NAWA decision granting scholarship – NAWA scholars only
Candidates for second cycle studies
Scans – to be submitted via IRK system
- Photo
- CV including your education and scientific interests- candidates of Quantum Information Technology only
- Diploma supplement, transcript of records or index – in the case of candidates for whom the Admission Committee decides on possibility to participate in further recruitment proceedings
- NAWA decision granting scholarship – NAWA scholars only
Paper documents
Candidates with ‘QUALIFIED’ or ‘RESERVE’ status who have received their education in Poland must deliver the original paper documents listed below within 4 days after the announcement of the list of candidates to the Admission Committee
- Self-signed personal questionnaire with photo generated and printed from the IRK system
- Diploma or proof of defense with final grade (in case of proof of defense candidate is required to submit the final diploma till October 31st to the Dean’s Office at the Faculty).
- Diploma supplement- in case of candidates applying for studies where grades average is one of the acceptance criteria or in case of candidates for whom the Admission Committee decides on possibility to participate in further recruitment proceedings
- Passport – in case of foreigners
- Legal representative statement of consent- in case of candidate under 18
- NAWA decision granting scholarship – NAWA scholars only
